COURAGE: Action for Better Aging survey

What is COURAGE? COURAGE: Action for Better Aging initiative is a social movement for change to reimagine aging where older adults have choice, independence and quality of life. COURAGE is building a coalition to mobilize action towards change. It is a partnership between SE Health and the Covenant Health family.

Who is this survey for? Older adults in Canada who live at home and in the community — in other words, outside of nursing homes and long-term care facilities.

What is the purpose of this survey? We want to understand the daily living and care goals and needs now and in the future. We would also like to learn about the challenges older people face to achieve these needs and goals.  The feedback will help the COURAGE team prioritize our activities to support a new vision for aging in Canada.

Option to enter a draw for a gift card: Participants have the option to enter a draw for 1 of 10 gift cards (each valued at $50). The odds of winning are based on the number of participants.

The survey is open until May 27, 2022.
