Brochures: The Thrive Project - Access to home and community care services

The Thrive team engages with older adults living with HIV about their health and access to home and community care services. The team has discovered that the information on these services needs to be more readily available to the public in one place. To meet this need, they have developed a brochure summarizing what home and community care services are and how to access those, based on the information provided by health authorities. In addition, together with peer navigators from the Positive Health Services clinic in Surrey, the Thrive team has developed a brochure on home and community care services for those living in the Fraser Health area, which has been also translated into Simplified Chinese, Punjabi, Arabic and Swahili.
