To support the shift in United Way BC Healthy Aging's Priority Populations, and in alignment with Healthy Aging’s new Program & Service Design Model, please find the Theory of Change (TOC) to assist agencies in service delivery to vulnerable seniors in BC.
This document was created based on robust consultations with the Community-Based Seniors’ Services (CBSS), the CBSS Celebration and Learning Event attendees, the Learning & Evaluation Advisory Committee, and the CBSS Leadership Council.
