At self management BC, we have the expertise in promoting health and wellness programs tailored to the senior community and further, want to bring forth a comprehensive frailty awareness initiative and self management strategy to prevent or slow down progression of frailty. Frailty is critical yet often overlooked aspect of senior health. Our Frailty Coach Program focuses on equipping residents, families, and caregivers with the knowledge and tools to identify early signs of frailty, prevent its progression, and promote healthier and more independent living. The program is grounded in evidence-based practices and has shown significant success in enhancing the quality of life for seniors.


The Self-Management Frailty Coach Program is a free, telephone-based program that supports adults in BC who are experiencing difficulty managing their frail condition and/or those wishing to prevent frailty.


The participants are mailed a copy of the Living a healthy life with chronic conditions free of cost to their home address along with AVOID strategy brochure, clinical frailty scale to self assess on the scale. The main aim of the program is to help the participant stay at the self assessed number, provide support and strategies to reverse back wards and prevent from going down the frailty scale. Also, participants are paired with trained frailty coaches for 13 weeks.


Frailty coaches connect with the participants by telephone once a week for approximately 30-45 minutes for 13 weeks. The frailty coach guides with self management strategies and introduces the AVOID strategy to create action plans or problem solve following the AVOID strategy gaining more insight about Diet and nutrition, isolation and social interaction, medication, vaccination, and activity or mobility.
