[Report] The Caregiver Rx Initiative – Evidence and Implementation for Caregiver-Focused Social Prescribing

This report highlights the first national initiative to design, implement, and evaluate social prescribing pathways for caregivers across Canada. this report demonstrates how connecting caregivers to community-based supports can improve mental, physical, and social well-being while strengthening integration between health and community care systems.

Key findings show significant improvements in caregivers’ well-being, access to support networks, and ability to navigate available services. The report also offers practical guidance for organizations looking to design, implement, and scale caregiver-focused social prescribing programs.

We are grateful for support from Canadian Centre for Caregiving Excellence and partnerships with Family Caregivers British Columbia, Caregivers Alberta, Ontario Caregivers Organization and Caregivers Nova Scotia for this initiative.
