[Toolkit] Serious Illness Conversation Implementation Toolkit

resource image

A new BCCPC resource can help make serious illness conversations (SICs) a routine part of care in ways that fit local needs and existing workflows. The SIC Implementation Toolkit is a five-part fillable resource designed for frontline clinicians, clinical champions, and leaders. It helps teams assess current practices, identify and prioritize local barriers, and develop practical, evidence-informed approaches to implementation. A supplementary guide provides additional background information, examples, and implementation support.

Join our webinar on October 26, 2026 for an introduction to the toolkit, including who it is intended for and how to use it effectively. Register here.
